
Eli Lilly secures obesity drug coverage across top three U.S. PBMs
Eli Lilly (NYSE:LLY) announced a major commercial milestone for its metabolic health franchise, securing comprehensive formulary placement from the three largest pharmacy benefit managers (PBMs) in the United States to cover its entire obesity medicine portfolio.
The agreements will significantly scale patient access to Lilly’s FDA-approved weight management treatments, bringing both its market-leading injectable therapy, Zepbound, and its recently approved daily oral pill, Foundayo, under the primary coverage umbrellas that dictate prescription costs for the vast majority of insured Americans.
The commercial rollout under the new coverage mandates will proceed on a phased timeline over the coming months.
For CVS Caremark, coverage for Foundayo is scheduled to begin on June 1, 2026.
In tandem, access for current and new Zepbound patients will expand systematically, with broader integration across the PBM's commercial template plans finalized by October 1, 2026.
The expanded access frameworks are structured to sharply lower consumer out-of-pocket hurdles, which have historically limited adoption in the high-demand anti-obesity therapeutic class.
Under the updated commercial insurance guidelines, eligible patients may pay as little as $25 per month for their prescriptions.
Lilly is also moving to bridge coverage gaps within government-sponsored plans, where federal statutes have historically restricted standard coverage for weight-loss medications.
Beginning July 1, 2026, the drugmaker will initiate a specialized "GLP-1 Bridge" program.
The initiative is designed to allow select Medicare Part D beneficiaries to access the portfolio therapies at a capped out-of-pocket cost of $50 per month.
The structural market expansion comes as competition intensifies within the global metabolic drug landscape.
Zepbound currently maintains its position as the most-prescribed injectable weight-loss medication in the domestic market.
By locking in universal top-tier PBM positioning alongside Foundayo—the first small-molecule oral GLP-1 treatment engineered without strict daily food or water consumption restrictions—Lilly establishes a highly competitive commercial moat across both the injectable and oral segments of the multi-billion-dollar obesity market.
